Project: Salud! Personal Data Collection and Analysis Platform

Purpose
The purpose of this study is evaluate Salud!, a web-based application that allows individuals to track and review data about their everyday activities. The study will help us understand how individuals integrate such a system into their everyday activities, gather feedback about how we can improve the system, and evaluate its effectiveness at supporting healthy changes in behavior.
Procedures
If you decide to be in this study, you will have the ability to use the Salud! application. You are free to use the application as often and for as long as you like. We ask that you provide us with any feedback you may have about your experience with the application. This includes the ways you are using it, problems you run into, requests for new features, and any other comments you may have. You may provide feedback as often as you like. We will log your use of the various features of Salud!. This log data will be aggregated and anonymized to establish usage statistics, diagnose errors or bugs, and develop new features. We will not view or share the content you upload and store in your Salud! account without your explicit permission.
Your participation in this study is completely voluntary and you may end your participation at any time.
Risks or Discomforts
The risks involved in this study are no greater than those involved in daily activities such as browsing the Internet or using common office software.
Benefits
You are not likely to benefit in any way from joining this study. However, we do hope that you find the application useful and enjoy using it. In the future, the results of this study may help to develop better and more useful applications.
Compensation to You
There is no compensation for participating in this study.
Confidentiality
Data collected about you and your privacy will be protected to the extent allowed by law. To make sure that this research is being carried out in the proper way, the Georgia Institute of Technology IRB may review study records. The Office of Human Research Protections and/or the Food and Drug Administration may also look over study records during required reviews. The following procedures will be followed to keep your personal information confidential in this study:
- Your name and any other fact that might point to you will not appear when results of this study are presented or published.
- The data you input into Salud! will be kept in a database on a secured computer at the Georgia Institute of Technology. Information transmitted between your browser and the Salud! server will be encrypted.
- The private contents of your Salud! account are kept confidential. Members of the research team may only review the contents of your Salud! account with your explicit permission.
- You will be promptly notified if any of your data is compromised by unauthorized individuals.
Costs to You
If you choose to use SMS (text) or MMS (multimedia) messages to interact with Salud!, your cell phone service provider may charge you a per-message fee. Please ensure that you are aware of any such charges before enabling SMS and MMS functionality in your Salud! account. Otherwise, there are no costs to you, other than your time, for being in this study.
In Case of Injury/Harm
If you are injured as a result of being in this study, please contact Dr. Elizabeth Mynatt at telephone (404) 894-7243. Neither the Principal Investigator nor Georgia Institute of Technology has made provision for payment of costs associated with any injury resulting from participation in this study.
Participant Rights
- Your participation in this study is voluntary. You do not have to be in this study if you don't want to be.
- You have the right to change your mind and leave the study at any time without giving any reason and without penalty.
- Any new information that may make you change your mind about being in this study will be given to you.
- You may print a copy of this consent form to keep for your records.
- You do not waive any of your legal rights by signing this consent form.
